The increasing number of older adults in Indonesia has contributed to the rising prevalence of chronic diseases, particularly hypertension and diabetes mellitus, which adversely affect the quality of life. This community service program aimed to enhance health literacy, assess functional status, and provide community-based physiotherapy interventions for participants of the Chronic Disease Management Program (Prolanis) at the Lisu Primary Health Center, Kading Village, Barru Regency. The program was implemented in two stages: health education and functional screening, followed by a structured exercise intervention. Health education was delivered through interactive sessions on cardiovascular, neuropsychiatric, and women's health. Functional screening included assessments of pelvic floor muscle function, cognitive function, handgrip strength, and measurements of triglyceride and long-term blood glucose levels. The interventions consisted of Kegel exercises, Brain Gym activities, and brisk walking, involving physiotherapy lecturers, physiotherapy students, and Prolanis community health volunteers. Of the 74 older adults enrolled, 57 completed the program. Screening results revealed variations in cognitive function, muscle strength, and metabolic status, indicating the need for individualized interventions to address these issues. The program received positive responses, as reflected by high participation rates among older adults and improved readiness of community health volunteers to independently facilitate future exercise sessions. This community-based physiotherapy program was effective in improving health literacy, providing baseline data on the functional status of older adults, and strengthening community capacity to support sustainable health management in the older people.
